The above information must be complete to qualify for a referral bonus.
Any referred candidate must be converted to a New Dimension HCS, LLC employee within six months of the referral date.
The referral bonus will be paid following 160 straight time hours worked with NDHCS.
Referral program is effective through December 31, 2008. All qualified referrals must be sent to NDHCS by the effective end date of the program.
Any candidate NDHCS is already working with is ineligible.
